INSTRUCTIONS for MANDATORY
Online Team Information/Code of Conduct/Media and Player Information/Roster Forms

Team Information/Code of Conduct and Player Information/Roster Forms will now be completed online at USYouthSoccer.org.  TEAMS (State Champions, Far West Regional League Wildcards, State Wildcard Teams) will access the forms via a link from the Region IV Website under the Regional section called NCS Register.  Please complete the Team information first, followed by the player information.

THESE FORMS ARE MANDATORY FOR ALL AGE GROUPS

The deadline for completing these forms is June 1, 2009

Team Information/Media/Code of Conduct

(If you have already created an account for USYouthSoccer.org, enter your User ID and Password.  If you have not, click Sign Up Now (top of page).  This will be your log-in information to go back and update information as needed.  You will automatically be taken to the Team Information page.  Once logged in you can update team and player information)

5.   Complete all information on this form.  To better prepare you, the following information is contained on this form:

1. General Team Info (Team Name, Region, State Association, Age Group, Gender and Uniform Color).
2. Coach Information (Name, Cell phone, email and address) – See Note below
3. Manager Information (Name, Cell phone, email and address) – See Note below
4. Assist Coach(es) and Trainer (Names only) – See Note below
5. Hotel Information
6. Emergency Contact Information – persons listed should be attending the event!!
7. Bench Pass Info:  ID#’s not necessary for Region 4
8. Roster Release:  If you wish to release your roster for college coaches and media use please √ this box
9. Media Information (i.e. team spokesperson, major newspapers etc, team background, interesting stories etc.  This will help in possible news stories about your team)

Note:  The Coach, Manager, Assistant Coach and Trainer information SHOULD match the Blue Roster!!!

6.   Click on the “Save” button and your information has been submitted!  If you need to return and edit the form you may do so.  To edit your team information –

Login, then click the Team Info link (http://www.usyouthsoccer.org/ncs_teams/index.asp)

Click on the Pencil icon to edit a team or click on the Players button to add/delete players on the team.

Under 12’s, U13’s, U14’s, U18’s, and U19’s:  Complete (a) through (g) ONLY.

Under 15’s, U16’s, U17’s:  Complete (a) through (g) AND (i) through (k).  Please see note regarding release of email address below.

1. Login to USYouthSoccer.org
2. Click on Player Profile link.
   
   (You will need to be logged into the USYouthsoccer.org website to add players to your team.  The same User ID and Password should be used as for the Team Information, same as you did above.  If you are adding multiple teams, use the same login information and select the team you are updating from the list of your teams.)
    
3. Complete the “Add a Championship Player” form for each member of the team.  To better prepare you, the following information will be required for this form:

1. First Name
2. Last Name
3. Jersey #
4. Position
5. Registration/ID number
6. Birth Date
7. Alt. Jersey #
8. Game Active (do not √ - National Championships only)
9. Grad Year
10. GPA, ACT, SAT
11. Email and hometown:  If you do not want email released, please leave blank.

To edit player information:

Login, then click the Player Info link (http://usyouthsoccer.org/ncs_teams/ncs_players.asp)

Click on the Pencil icon to edit a player or the Team button to add/edit team information.

Click on the save button after you have completed each player’s record.  If you need to return to edit you may do so.
