POLICIES AND PROCEDURES

I.  PURPOSE:

To preserve the history of our organization and honor those individuals who have made outstanding contributions to US Youth Soccer REGION IV in any of the following capacities:

• Player
• Coach
• Referee
• Administrator

II.  COMMITTEE ORGANIZATION:

A.  The Director of USYSA REGION IV shall charter a US Youth Soccer REGION IV Hall of Fame Committee and appoint the Chairman of the Committee. The Committee shall be comprised of the following:

• Chairman;
• No less than five committee members selected from the State Associations of Region IV.

B.  The committee members should have a thorough knowledge of the history of youth soccer in the state they represent, US Youth Soccer REGION IV, and the game at all levels.

III.  USYSA REGION IV DIRECTOR RESPONSIBILITIES:

Prior to September 1 of each year, the US Youth Soccer REGION IV Director shall review the following:

A.  Deadline for submission of that year's nominations to US Youth Soccer REGION IV is 30 January;

B.  Deadline for the committee to finalize its decisions for that year's nominations is 15 May;

C.  Date, time & place for presentation of that year's inductees into the US Youth Soccer REGION IV Hall of Fame.

IV.  SELECTION PROCESS FOR NOMINATIONS:

A.  Nominations for consideration must be submitted by one of the following:

• State Association
• Region IV Committee
• Region IV Board

B.  Individual candidates may be nominated up to three (3) times. If not selected after three (3) nominations, they will not be given further consideration unless nominated specifically by the US Youth Soccer REGION IV Board of Directors.

C.  The nominee's resume shall be concise and reflect the chronology of major contributions and achievements.

D.  All resumes shall be typed on the US Youth Soccer REGION IV Hall of Fame Application forms.

E.  Completed nomination forms shall be sent to the US Youth Soccer REGION IV office, and received no later than the date established by US Youth Soccer REGION IV per item III.A. The nomination form shall be recorded at the office and copies shall be sent to the Hall of Fame Committee for review. The original nomination forms will be filed in the US Youth Soccer REGION IV Office.

F.  The Committee will only accept resumes received by the US Youth Soccer REGION IV office and which conform to the format specified in IV.D and meet the deadline established by US Youth Soccer REGION IV per item III.A.

G.  The Committee shall make its decision on all nominees no later than the date established by US Youth Soccer REGION IV per item III.B.

V.  CRITERIA FOR SELECTION:

A.  The US Youth Soccer REGION IV Hall of Fame nomination should reflect all contributions to youth soccer with particular emphasis on the individuals contribution to US Youth Soccer REGION IV.

B.  Eligibility: To become eligible for nomination to the US Youth Soccer REGION IV Hall of Fame, an individual must have a minimum of five (5) years in service to US Youth Soccer REGION IV.

C.  Selection:

1.  For the first three (3) years that nominations are accepted (Jan 2000 - Jan 2002):

• The US Youth Soccer REGION IV Hall of Fame Committee may select a maximum of six (6) individuals (living or deceased) each year to the US Youth Soccer REGION IV Hall of Fame, or it may choose to select none, if it is felt that the nominees proposed do not meet the qualifications.

2.  For the subsequent years (Jan 2003 and beyond):

• The US Youth Soccer Region IV Hall of Fame Committee may select a maximum of three (3) individuals (living or deceased) each year to the US Youth Soccer REGION IV Hall of Fame, or it may choose to select none, if it is felt that the nominees proposed do not meet the qualifications.

VI.  NOTIFICATION PROCEDURES:

The US Youth Soccer REGION IV Hall of Fame Committee Chairman shall:

A.  Notify the US Youth Soccer REGION IV Director the committee's decisions;

B.  Notify the Inductees and/or their families of their selection to be inducted into the US Youth Soccer REGION IV Hall of Fame;

C.  Issue a news release announcing the selections for induction into the US Youth Soccer REGION IV Hall of Fame.

VII.  INDUCTION & AWARDS:

A.  Induction of newly selected Hall of Fame members shall take place at a time and place established by US Youth Soccer REGION IV per item III.C.

B.  Each individual inducted into the Hall of Fame shall receive a certificate and suitable award.

VIII.  RECORDS:

A.   The US Youth Soccer REGION IV Office will maintain a copy of all properly submitted nominations.

B.  The Hall of Fame Committee will forward the results of its work (review of nominations) to the US Youth Soccer REGION IV Office for historical record-keeping purposes.

C.  These records will be maintained to ensure compliance with item IV.B.

D.  US Youth Soccer REGION IV shall maintain a biographical library (including pictures if possible) of all US Youth Soccer REGION IV Hall Of Fame Inductees.
